License Renewal
Renewal of your license
should be completed prior to October 15, 2009. Your renewal form
and check in the amount of $150.00 must be mailed to the Board
office and your continuing education requirement met in order
for your license to be renewed. Credit card payments are not
accepted. The renewal form may be printed by clicking
